Make sure your full name as given in your credit report, address, social security number, copy of your driver's license, copy of credit report, and copy the evidence, ie canceled check.

Make sure a copy of whatever you need for each Credit Bureau. Note the day that you mail your letter to info from credit report. By certified or registered mail is a good idea because they will be proof of mailing date.
